Well a lot of it has to do with brushes.
You download them onto your computer and that's what makes the paint splatters, the hearts, the stars, etc.
As for simple layouts like my pacman one for instance...
Just use the shape tool and draw stuff.

and sorry i have to go to a wedding party in the middle of nowhere and the cows are trated badly and its all dirty and huge smellwaves of cow manure every 5 minutes
